How Do I Obtain My Medical Records?
The Medical Records Custodian at Riverside Women’s Care fulfills requests for information for medical records for both the Beaufort and Bluffton locations. Our Medical Records Custodian will be happy to assist you in obtaining your medical records upon receipt of a valid authorization. You can mail, fax, or personally deliver your authorization to release health information.
NOTE: Forms must be filled out completely to be considered valid. Legal identification is required to obtain your medical records.
- Mail: Print the Riverside Women’s Care HIPAA-compliant authorization form and mail it, along with a copy of legal identification, such as a driver’s license, to:

- Fax: Fax the Riverside Women’s Care HIPAA-compliant authorization form to 843-524-5655, including a copy of legal identification, such as a driver’s license.
- Front Desk: You are welcome to stop by either of our offices at the addresses below. The staff can assist you with requesting your records. Office hours are Monday through Thursday, 8:30 a.m. to 4:30 p.m. and Fridays 8:30 a.m. to 2:00 p.m. Questions? Our number is 843-540-5857.
